Instructions to Trainer:

1.  All individuals trained to do DOT and/or forensic collections must pass an examination and flawlessly complete five mock collections.  Note below exam score and pass or fail on mock collections.

2.  All trained collectors must take a refresher course at least every five years.

3.  Collectors who cause a fatal flaw during collection must take error-correction training and flawlessly complete three mock collections.  One must be uneventful, the other two in the area in which they caused the fatal flaw.   They do not have to take an examination.  Only note pass or fail in the mock collections.
